Forum in English / Documentary on Lem (SOLARIS, DVD, Criterium)

I'm looking for the WHOLE documentary that appears in the Criterium Collection edition of SOLARIS by Tarkovski. It's in Polish, with English subtitles. The DVD has only 5 minutes of it.
Or at least a reference (when it was made, who produced it, etc).
